Who We Partner With
We work alongside the full spectrum of healthcare — from solo-practice PCPs to large health systems — to ensure patients get psychiatric care without falling through the cracks.
Primary Care Physicians
PCPs are on the front lines of mental health. We accept warm referrals from primary care providers and coordinate care seamlessly — sending notes back within 48 hours of each visit.
Therapists & Counselors
We collaborate with licensed therapists to provide psychiatric medication management for your existing clients. You handle therapy; we handle the medication evaluation and prescribing.
Hospitals & Health Systems
Reduce psychiatric boarding and readmissions by partnering with Hometown NP for step-down telehealth psychiatric care. We can see discharged patients within days, not weeks.
Employers & EAP Programs
Offer your employees access to board-certified psychiatric care as part of your benefits package or employee assistance program. We accept most major commercial insurance plans.
Community Mental Health Centers
We partner with FQHCs and community mental health centers to extend PMHNP capacity into underserved communities. Telehealth removes the access barriers your patients face every day.
Schools & Universities
College counseling centers and K–12 programs can partner with us for psychiatric consultations and co-management of students needing medication evaluation alongside counseling services.
